Reverse total shoulder arthroplasty is a surgical procedure used to relieve pain in patients suffering from severe arthritis due to rotator cuff tears. However, problems such as implant–bone impingement or joint instability have been reported postoperatively. To address these issues, designs incorporating lateral offset by modifying the prosthesis geometry have been introduced, but debate over their clinical outcomes continues. Particularly in laterally offset reverse shoulder arthroplasty, the impact on implant–bone impingement or joint stability based on the degree of the patient's rotator cuff tear remains unclear. Therefore, biomechanical analysis based on rotator cuff tear conditions and implant type is necessary to establish surgical guidelines that can improve clinical outcomes for patients.
This study suggests that restoring the subscapularis during lateralized reverse total shoulder arthroplasty can reduce implant–bone impingement and joint subluxation, even in the presence of a posterosuperior rotator cuff tear.
